What the Research Shows
Cesarean section delivery is one of the most common surgical procedures worldwide. While many C section scars heal well, some people experience visible scarring, thickened tissue, tightness, or uneven texture along the incision line.
Treatments that support collagen remodeling and skin regeneration may help improve the appearance and flexibility of surgical scars. One treatment that has gained increasing attention in dermatology and aesthetic medicine is microneedling.
Research suggests that microneedling may be an effective, minimally invasive option for improving surgical scars, including C section scars.
What is Microneedling?
Microneedling is a procedure that uses very fine sterile needles to create controlled micro injuries in the skin.
These small, controlled injuries trigger the body’s natural wound healing response, stimulating the release of growth factors and encouraging new collagen production. This process is sometimes referred to as percutaneous collagen induction in research.
As the skin heals, collagen remodeling can help improve:

Evidence for Microneedling in Surgical Scars
Research supports microneedling as an effective treatment for surgical scars.
Studies have shown 50 to 75 percent improvement in scar appearance when microneedling is used alone or in combination with other therapies.
One randomized study evaluating C section scars and other surgical scars found that combination therapy using microneedling and silicone gel produced the greatest improvement.
Results from that study showed:
68 percent improvement with microneedling plus silicone gel
52 percent improvement with microneedling alone
47 percent improvement with silicone gel alone
These findings suggest that microneedling can be a valuable component of scar treatment, particularly when combined with other supportive therapies.
When Should Microneedling Be Started?
Timing appears to influence treatment outcomes.
Research examining postsurgical scars found that starting microneedling earlier in the scar maturation process produced better results.
In one study, treatments initiated 6 to 7 weeks after surgery resulted in significantly better aesthetic outcomes than treatments started later at 13 to 16 weeks.
This may be because the scar is still undergoing active collagen remodeling during early healing.
However, even older scars may still benefit from microneedling treatments.
How Many Treatments Are Needed?
Microneedling for scars is usually performed as a series of treatments.
Typical treatment protocols involve 2 to 6 sessions spaced about one month apart.
Large prospective studies have found that many patients experience at least 50 percent improvement after an average of about 2.5 treatments.
Gradual improvement continues as collagen remodeling progresses between sessions.
Safety and Recovery
Microneedling is generally considered a safe and well tolerated treatment across all skin types.
Compared with more invasive procedures such as laser resurfacing, microneedling has a relatively low risk of complications.
Common temporary effects may include:
redness
mild swelling
skin sensitivity
These effects typically resolve within a few days.
